Introduction

The etiological spectrum of disease affecting the quadriceps femoris muscle includes the myopathies (dystrophic, myositic, and acquired), neurogenic affections (anterior horn cell or nerve), and possibly a combination of neurogenic and muscular disease. The diagnosis is important because of the high frequency of involvement of the quadriceps femoris in the primary diseases of muscle, the occasional presentation of quadriceps muscle weakness as an early sign of an endocrine or metabolic disturbance or of disease of the anterior horn cells, and the frequently unrecognized involvement of the femoral nerve in a mononeuritis or as part of polyneuritis.

Muscle biopsy and routine electromyography may be of considerable aid in differentiating primary diseases of muscle from the neurogenic diseases but are less helpful in specific differentiation, and are by no means infallible.
