On behalf of the American Board of Psychiatry and Neurology (ABPN), I am pleased to reply to Menken. He implies that inasmuch as the Neurology Directors of the ABPN practice in academic medical centers, their patient population consists of "acute, complex, and esoteric problems" rarely encountered by private practitioners. He suggests that the composition of the Board should represent the "demographic attributes and patterns of responsibility" of candidates for certification.

As part of their attending and consultation duties, the eight Neurology Directors see patients with a wide variety of neurological diagnoses, including headaches, backaches, and epilepsy. As ambulatory care has expanded in academic medical centers, an increasing proportion of time is devoted to the evaluation and treatment of patients with common neurological problems.
