This third multiauthored edition has been extensively revised. Ten chapters have new authors. It is a challenging volume for practitioners and neurosurgical trainees with current and extensive references for all chapters. Two chapters are less extensive in their surveys of options and outcomes. Many are outstanding. It would, however, be difficult to select chapters of greatest achievement without doing an injustice to others. Despite the fact that there are 34 authors and 23 chapters, the style is remarkably clear, cogent, and focused throughout. Contrast studies, charts, and graphs are pertinent. The halftones of diagnostic studies are superior.

While no one reference volume, concerned with a global survey of material pertinent to a subject that is still in evolution, can resolve all contingencies of clinical practice preferences and the related basic sciences, this volume is a remarkable example of what can be achieved in this regard.
