To the Editor.

—Drs Ferro and Kertesz¹ presented an interesting case of neglect associated with infarction of the posterior internal capsule. To further elucidate, I wish to point out that their patient actually has an occlusion of the anterior choroidal artery. The territory matches the anatomic template previously noted by Damasio.² The syndrome has also recently been demonstrated and studied in two articles by French researchers.^3,4 When this syndrome occurs in the minor hemisphere, neglect can and does occur, presumably for the reasons elegantly outlined in the article by Drs Ferro and Kertesz.

The anterior choroidal artery syndrome, formerly an entity whose very existence was controversial, is now gaining increased attention and becoming much better understood.
