To the Editor.—

Benign orgasmic cephalalgia is a well-described entity, most likely of vascular origin, that occurs with a male preponderance. Previous reports have dealt with this type of headache occurring during sexual intercourse, and some authors believe that the preorgasmic relationship is a contributing factor.^1,2 We describe the case of a woman in whom typical orgasmic cephalalgia developed during masturbation; to our knowledge, this association has not been previously reported.

Report of a Case.—

A 28-year-old woman suffered a sudden throbbing occipital headache as she became orgasmic by masturbation. The cephalalgia lasted an hour and was associated with mild nausea, but there were no other associated neurologic symptoms. She had not experienced prior cephalalgia with or without sexual activity.
