In reply

We wish to thank Drs Mandava and Kent for their comments on our recently published article. They express several concerns about the findings of the systematic review.

Their first concern regards the use of case reports and case series in this systematic review. We agree that there are limitations of this study and are explicit in our article, outlining that of the 114 articles identified, 91 were excluded as they did not contain any original cases or data. Of the 23 articles that remained, 1 was excluded as it also lacked patient data. Finally, of the remaining articles, 14 case series and 8 case reports were identified and used for this analysis.
