Neuromyelitis Optica and Autoimmune Diseases

The recent article published by Pittock et al1 proposes that the association between neuromyelitis optica (NMO) spectrum disorders and other autoimmune diseases such as Sjögren syndrome (SS) or systemic lupus erythematosus represents 2 independent conditions that coexist in the same patient. We describe a patient with clinical, serologic, imaging, and neuropathologic findings that support their notion; our patient underwent anti-aquaporin 4 antibody testing that was found positive at a dilution of 1:1920.

A 43-year-old female patient with a history of dry mouth, dry eyes, and 1 episode of parotiditis had positive titers for SS-A, SS-B, and antinuclear antibody titers of 1:1280. A lip biopsy specimen showed lymphocytic infiltrates of the salivary glands and the SS diagnosis was made.
