Spatial Disorientation in Alzheimer's Disease

Abstract

• Spatial disorientation was investigated in 28 ambulatorypatients meeting the National Institute of Neurological Disordersand Stroke-Alzheimer's Disease and Related Disorders AssociationWork Group criteria for "probable" Alzheimer's disease. Basedon caregivers' reports, 39% of subjects engaged in at leastthree of four behavioral measures of spatial disorientationthree or more times a week; these patients did not significantlydiffer from other Alzheimer's disease subjects with regard toage, sex, education, or symptom duration. Using stepwise regressionanalysis, we found that neuropsychologic measures of memoryand visuoconstructive functions, but not disease severity, attention,or language impairment, emerged as significant predictors ofspatial disorientation. In the setting of impaired memory, thetendency of some patients with Alzheimer's disease to wanderor to get lost may implicate particularly severe dysfunctionof right hemisphere neocortical areas concerned with visuospatialprocesses.
