Reproductive Endocrine Disorders in Women With Partial Seizures of Temporal Lobe Origin

Abstract

• Of 50 consecutive women with partial seizures of temporallobe origin (temporal lobe epilepsy [TLE]) evaluated for reproductivedysfunction, 28 had menstrual problems. Of those, 19 had reproductiveendocrine disorders. Polycystic ovarian syndrome and hypogonadotropichypogonadism occurred significantly more often in women withTLE than in the general female population. Polycystic ovariansyndrome was associated with predominantly left-sided lateralizationof interictal epileptic discharges; hypogonadotropic hypogonadismwas more commonly found with right-sided discharges. Hyposexualityoccurred more often in women with predominantly right-sidedinterictal epileptic discharges and was associated with lowserum luteinizing hormone levels. There are several possibleinterpretations: (1) epileptic discharges in medial temporallimbic structures may disrupt hypothalamic regulation of pituitarygonadotropin secretion; (2) anovulatory cycles of reproductiveendocrine disorders may promote the development of epilepticdischarges; and (3) TLE and some associated reproductive endocrinedisorders may represent the parallel effects of prenatal factorscommon to the development of the brain and the reproductivesystem.
