Cervicocephalic Arterial Dissections

A Ten-Year Experience

Abstract

•Between April 1976 and April 1986, cervicocephalic arterialdissections were diagnosed in 19 of 4531 patients undergoingcerebral arteriography for acute cerebrovascular symptoms. Mostdissections were extracranial. Spontaneous cases were as frequentas those following known trauma. Cerebral infarction was themost common presentation. Thirteen patients received only medicaltherapy, and six had surgery. Most patients survived the initialinsult. Many questions remain to be answered regarding the optimalmanagement of this condition, but treatment should be individualizedbased on clinical status, computed tomography, and angiographicfindings.
